Household employee definition: Work performed for someone at their home and he/she controls what work is done and how work was performed.


*Examples of household workers: Babysitters, Caretaker, Cleaning people, Domestic workers, Drivers, Health Aides, Housekeepers, Groundkeepers, Maids, Nannies, Private Nurses, Yard Workers, etc.


If any taxes were withheld, the employer is required to provide the tax preparer with a W-2. If no W-2(s) was provided, give the total amount you were paid from all employers combined for the year.


You must list all household employers. If you have multiple employers that did not provide a W-2, complete this form for each employer.
